OUR GOALS AND OBJECTIVES

 

RESCUING HEARTS

​

Goal 1: Blood pressure measurement

Initiative: Conduct community screenings for high blood pressure and establish partnerships with healthcare facilities, pharmacies, and local communities to expand screening availability. Advocate for the use of home blood pressure monitors to enable individuals to take charge of their cardiovascular health.

​

Goal 2: Community Cardiovascular Risk Assessment

Initiative: Organize community-based cholesterol testing events, offering screenings for total cholesterol, LDL (bad) cholesterol, HDL (good) cholesterol, and triglycerides. Utilize blood samples collected during these events to assess cardiovascular risk and promote early intervention for at-risk individuals within the community.

Goal 3: Diabetes Risk Screening and Management

Initiative: Conduct blood glucose screenings within the community to detect individuals at risk of diabetes, with a focus on those with risk factors like obesity or family history. Provide targeted education on diabetes prevention and management to reduce associated risks, including heart disease.

Goal 4: Tobacco Use Screening and Smoking Cessation Support

Initiative: Implement comprehensive tobacco use screening and cessation support programs to reduce cardiovascular risk associated with smoking. Offer interventions such as counseling and support programs for smoking cessation to help individuals reduce their cardiovascular risk and improve heart health.

Goal 5: Facilitating Access to Comprehensive Cardiovascular Assessments

Initiative: Establish partnerships with clinics to refer patients requiring further cardiovascular evaluation for comprehensive checks, ensuring access to necessary assessments and interventions for optimal heart health.

PRESERVING SIGHT

​

Goal 1: Vision Screening

Initiative: Detect poor vision or risk factors hindering normal visual development early for better outcomes.

Initiatives:

  1. Cataract Screening: Perform free cataract surgeries for individuals diagnosed with cataract.

  2. Glaucoma Screening: Refer glaucoma suspects for expert examination to prevent blindness; provide free medication for diagnosed patients.

  3. Macular Degeneration Screening: Offer free ocular antioxidant medication and referral for further management based on stages of macular degeneration.

  4. Diabetic Retinopathy Screening: Counsel individuals with diabetic retinopathy and refer them to appropriate centers for further care.

Goal 2: Ocular Condition Treatment

Initiative: Provide free ocular medication for minor ocular conditions, including allergies, vernal, atopic, and infectious conjunctivitis, dry eye, and other ocular surface disorders.

Goal 3: Preventing Blindness and Vision Impairment

Initiatives:

  1. Provide free refraction services and dispense free glasses to school children.

  2. Prescribe free glasses for presbyopes, addressing age-related difficulties in near reading.

Goal 4: Low Vision Services

Initiative: Provide comprehensive low vision services, including evaluations, training on low vision devices, and counseling, to rehabilitate visually impaired individuals.

Goal 5: Promoting Ocular Health Awareness

Initiative: Educating individuals on maintaining optimal vision health.

​

Goal 6: Continuity

Initiative: Train community focal points to act as advocates for sight rescue, enabling them to identify common eye diseases post-project and refer individuals for appropriate care.
